Milwaukee Worker’s Compensation – Superior Company Cited By OSHA After Employee Killed In Accident

The Occupational Safety and Health Administration cited a Superior, Wis. company for nine safety violations following the May death of an employee at their facility. OSHA cited White Cedar Shingles Inc. for nine safety violations and is facing fines of $156,240 for conditions found after the employee was killed May 21 while servicing machinery.  According to the OSHA press release, the machinery had not been locked out to prevent an unexpected startup.

“White Cedar Shingles was cited for the same deficiency in 2012, and that underscores its failure to act. Employers must identify and correct hazards and ensure workers follow proper procedures to prevent injury or death,” OSHA area director Mark Hysell said.

OSHA placed the company in the Severe Violator Enforcement Program due to the repeat violations. The company received violations for failing to train workers authorized to perform service as well as failing to control electrical energy sources by installing lockout/tagout devices.

The company is subject to mandated follow up inspections to ensure the company complies. The company also received five serious violations including failure to train and evaluate forklift operators.
